The number of tables may be limited; therefore requests will be taken on a first-come, first-served basis.  Please submit this form as soon as possible to reserve your space. Meals are not included in exhibit fee.
**All meals have to be pre-ordered, there is an additional cost of $55.00. This includes Wednesday & Thursday luncheons and Banquet on Thursday evening.
 
You will need to confirm your table/space prior to September 30, 2009. Only paid vendors will be allowed to set up at conference.
